2011 Forms


A - Photo Safari DVD Photo List (PDF version)

This form will be used to label all the photos you take.

  • First column (Order of Photo on DVD) is the order that you put your photos onto the DVD.
  • Second column (Photo Clue #) will be the clue # for each photo.
  • Third column (Describe the location etc.) is a description of where you took that photo.

Write down any information you feel important for you to get credit. Write clearly; if we can't read it, we can't score it. Make sure you fill out the identifying information at the bottom of the page.


B - Evaluation (PDF version)

There are 2 questionnaires. One is for the Scouts to fill out and the other is for the adult leaders to fill out. You will earn 75 points for each one completely filled out.


C - Activity Scoring Sheet

This is a sheet to record the activities that the Patrol completes. This form will be filled out by the Adult Leader.

This form will not be available until you get your clues online. At that time you can download and print.


D - Patrol Sign-In Sheet (PDF version)

This form is the list of all the Scouts and Adults participating at any time during the weekend of the event. You must send a copy of this form via e-mail to pauloliver@roadrunner.com before the contest starts. You can adjust this form at any time until the start of the event if you have changes - just e-mail an updated form.


E - Patrol Budget (PDF version)

This form is only for older Scouts (Boy Scout troops, Venture crews, Girl Scout Cadette / Senior / Ambassador troops). Younger Scouts (Cub Scouts, Girl Scout Brownie / Junior troops) do not need it.

This form is for the Patrol to keep track of all their expenses during the event. See the rules for what does and doesn't count.
